The mechanism is real
Icariin, the principal flavonoid in Epimedium species, does inhibit phosphodiesterase type 5 — the same enzyme targeted by sildenafil and tadalafil. This has been demonstrated repeatedly in cell and tissue preparations, and derivatives of icariin have been synthesised specifically to increase that potency.
The problem is magnitude and delivery. In-vitro comparisons put icariin's PDE-5 inhibitory potency at roughly 1/80th that of sildenafil. Icariin also has poor oral bioavailability — it is a glycoside that must be deglycosylated by gut bacteria before absorption, and plasma concentrations after oral dosing are low.
Multiplying a weak inhibitor by poor absorption gives a plausible explanation for why a mechanism that works in a test tube has not produced a convincing clinical result.
What human evidence exists
Very little. There are no adequately powered randomised placebo-controlled trials of epimedium for erectile dysfunction in men. What exists is largely animal work — rat models of erectile function and nerve injury, where icariin injections show effects — plus in-vitro studies and traditional-use documentation from Chinese medicine spanning centuries.
There is a somewhat better-developed literature on icariin and bone density in postmenopausal women, where epimedium-derived flavonoids have been studied for osteoporosis, again mostly in small trials.
Why this ingredient carries unusual risk
Because icariin's real effect is weak but its marketed effect is strong, products in this niche have a documented history of adulteration with undeclared sildenafil, tadalafil or their analogues. Regulators including the FDA and multiple European authorities issue recalls of sexual-enhancement supplements found to contain these compounds on a routine basis.
This is not a theoretical concern. An undeclared PDE-5 inhibitor is dangerous specifically for the people most likely to buy such a product: men on nitrate medication for angina, for whom the combination can cause a catastrophic drop in blood pressure. If a "herbal" product produces an effect comparable to a prescription drug, the most likely explanation is that it contains one.
Epimedium itself may also affect bleeding and blood pressure, and high doses have been associated with tachycardia, hypomania and respiratory arrest in isolated case reports.
