How Nitric Oxide Works in the Body
A gas with a half-life of a few seconds controls blood pressure, exercise capacity and erectile function. Understanding the two routes to producing it explains most of what circulation supplements are trying to do.
Nitric oxide is the signalling molecule behind vasodilation, exercise capacity and erectile function. Its production is measurable, it declines predictably, and it responds to specific dietary inputs.
The endothelium — the single-cell lining of every blood vessel — is a active endocrine organ with a combined surface area of several thousand square metres. Its most important product is nitric oxide (NO), generated from the amino acid L-arginine by endothelial nitric oxide synthase (eNOS). NO diffuses into the surrounding smooth muscle, raises cyclic GMP, and causes relaxation and vessel widening.
Endothelial dysfunction — reduced NO availability — is the earliest measurable step in atherosclerosis, appearing years before any visible plaque. It is driven by oxidative stress, hypertension, smoking, high LDL, insulin resistance and age, and it is at least partly reversible.
The L-arginine–eNOS pathway is oxygen-dependent and the dominant route at rest. Supplemental L-arginine feeds it, but oral arginine is largely destroyed by arginase in the intestine and liver before it reaches circulation. L-citrulline avoids this: it passes through the gut untouched, is converted to arginine in the kidney, and raises plasma arginine more effectively than arginine itself, gram for gram.
The nitrate–nitrite–NO pathway is independent of eNOS and works better under low oxygen. Dietary nitrate from beetroot, rocket, spinach and other leafy greens is reduced to nitrite by oral bacteria, then to NO in tissue. This is why beetroot juice has a reproducible effect on blood pressure and exercise economy in controlled trials — and why antibacterial mouthwash measurably blunts that effect by killing the oral bacteria that perform the first step.
Dietary nitrate (beetroot). Consistent small reductions in systolic blood pressure and improved exercise efficiency across multiple randomised trials. Effective dose is roughly 300–600 mg nitrate, which is one concentrated beetroot shot or a large serving of leafy greens.
L-citrulline. 3–6 g daily raises plasma arginine and NO markers; smaller but real effects on blood pressure and exercise performance. Better tolerated than high-dose arginine, which commonly causes gastrointestinal upset.
Polyphenols from cocoa flavanols, pomegranate and grape seed improve flow-mediated dilation in trials — a direct measure of endothelial function.
Exercise remains the single most effective endothelial intervention. Shear stress from increased blood flow directly upregulates eNOS expression. Nothing in a capsule matches it.
Smoking is the most direct antagonist: it generates superoxide, which reacts with NO to form peroxynitrite, simultaneously destroying NO and generating a damaging oxidant. Uncontrolled hypertension, high LDL, insulin resistance and chronic sleep deprivation all impair endothelial function measurably. Sedentary behaviour does so within days — bed-rest studies show flow-mediated dilation declining in under a week.

For raising plasma arginine and NO markers, yes — because oral arginine is largely metabolised before it reaches the bloodstream while citrulline is not. Trials comparing them directly find citrulline produces a higher and more sustained plasma arginine level from a smaller dose.
For blood pressure and exercise economy, the evidence is unusually consistent for a food-based intervention. The effect is modest — typically 3–5 mmHg systolic — and depends on oral bacteria converting nitrate to nitrite, which is why antibacterial mouthwash blunts it.
Flow-mediated dilation improves measurably within two to four weeks of regular aerobic training, and declines just as quickly with inactivity. It is one of the more responsive physiological markers available.
